Therapy Disclosures
The following disclosures, policies and documents ("documents") relate to your Patient Consent and Information Form signed when you first attended Reform ("the business").
By signing the Patient Consent and Information Form you have indicated that you have read, understood, accept all information outlined below and acknowledge that the information below may be updated or changed from time to time.
If we decide to make material changes to any of our documents, we will do our best to notify you by e-mail through the primary e-mail address specified in your account and/or post those changes to this page listed on our Website prior to the changes taking effect. You are responsible for ensuring we have an up-to-date active and deliverable e-mail address for you.
You are also responsible for regularly reviewing the our documents. We reserve the right to modify these documents at any time. No amendment to or modification of these documents will be binding unless in writing and signed by a duly authorized representative of Reform, or posted to the Site by a duly authorized representative of Reform.
There are certain risks (financial, physical, environmental, mechanical, etc.) associated with participating in any form of physical activity.
By participating in any form of physical activity or therapy with Reform, you acknowledge that you may be exposed to the risk of injury, inside or outside of the business and at times from an event resulting in catastrophic injury.
We recommend that all participants seek clearance from their general practitioner to gain medical clearance before undertaking any form of physical activity with the business. If you do decide to take part in any physical activity with Reform you understand that you may withdraw at any time and that you are under no obligation to attend any other sessions.
By signing the Patient Consent and Information Form, you confirm that you accept all risks associated with participating in any physical activity offered by Reform and hereby indemnify and release the business, activity leader, trainer, their agents, affiliates, employees, members, volunteers, sponsors, promoters and any other person directly or indirectly associated with the business against all liability (including liability for their negligence and the negligence of others), claims, demands and proceedings arising out of or connected with the participation in any prescribed or voluntary activity offered.
You may withdraw this consent at anytime in the future however please note that some components are irrevocable; for example, we are legally require to keep a copy of your client record even if you withdraw consent and cease treatment.
Confidentiality, Privacy and Record Management
Your treatment with us is confidential and private. This means we will not disclose the information you provide to us to Third Parties unless you consent for us to do so.
It is important to note that there are limits to confidentiality in exceptional circumstances including:
1. When there is a risk to you or others,
2. If we are responding to a subpoena or court order and
3. In the event that you consent for your details to be sent to another service provider.
Our staff will discuss this with you during your first appointment and answer any questions you might have.

Client Record Management: We will create and keep records for you which will record all relevant information relating to the counselling provided to you. We may also collect and store health information about you from Third Parties such as your doctor or general practitioner. Your records are managed and protected in accordance with all applicable local laws. Client records are legally required to be stored for 7 years following the conclusion of your counselling with us. The details of this can be found in our Privacy Policy.
You are also able to request access to a copy of your client record. Please discuss this with the business should you wish to do this at any time.
Personal Training and Pilates
When working with our coaches and trainers you will undertake activities that will will expose you to certain risks. By participating in such activities with a coach or trainer I may be injured, physically or mentally, or may die, my personal property may be lost or damaged, other persons participating in such activity may cause me injury or may damage my property, I may cause injury to third persons or damage their property, the conditions in which the activity is conducted may vary without warning, I may be injured or die or suffer damage to my property as a result of negligence or breach of contract of reform or affiliated trainers, there may be no or inadequate facilities for treatment or transport of me if I am injured, I assume the risk and responsibility for any injury, death or property damage resulting from my participation in the activity and agree to release, indemnify and hold harmless Reform from and against all and any actions or claims which may be made by me or on behalf of by other parties for or in respect of arising out of any injury, loss damage or death caused by me or my personal property whether by negligence, breach of contract or in any other way whatsoever.
Physiotherapy, Osteopathy and Massage
Our physiotherapists and osteopaths use a variety of manual techniques, and orthopaedic and functional tests which can help diagnose your specific injury, including: structural assessment, full patient history & treatment goal setting, deep tissue massage, mobilisations, HVLA (high velocity low amplitude manipulation), dry needling and cupping, contrast hot/cold therapy, exercise rehabilitation.
Manual therapy is recognised as being an effective and safe method of care for many conditions however there are a number of risks involved. If you'd like to know more about the risks, please ask the practitioner to elaborate further.
Counselling and Psychology
Nature of Services: All counsellors at Reform are fully qualified and registered with The Australian Counselling Association. Prior to commencing counselling your counsellor will discuss their proposed approach with you and ensure you understand the rationale for the approach, including any identified risks.
Limitation to Services: Counselling is provided via prearranged, scheduled appointments. Counsellors have limited availability for contact outside of scheduled appointments. Counsellors may not have an opportunity to respond between session contact prior to your next appointment so this should not be relied upon for emergency matters. For emergency matters please contact the appropriate agencies using the details below.
Counsellors will only provide services that are within their professional capabilities, competence and role as a private provider. If treatment needs are identified that are outside of this scope your counsellor will endeavour to assist you identify alternative suitable providers.
Emergency Contacts: We are not an emergency service. We are not able to respond to urgent or emergency matters and our communication channels (Phone, SMS, Fax, Email) are not monitored outside of standard business hours.
For urgent concerns relating to your health or other emergencies please contact the appropriate emergency services in your area.
Naturopathy and Dietetics
The form of health care provided at this clinic is based on scientific evidence-based Naturopathic Medicine, Holistic Nutrition and other supportive principles and practices. All information I provide to my practitioner about my current health condition is true and correct to the best of my knowledge. In the interests of safety, I agree to inform my practitioner of any changes to my health status, including any new pharmaceutical medications, natural medicines, injuries or health concerns.
Female patients only. I agree to inform my practitioner immediately if I am pregnant, planning to become pregnant, or breastfeeding.
Therapies undertaken in conjunction with my practitioner may involve risks and/or side effects, which I agree to discuss with my practitioner. Although my practitioner will check for known medicine interactions and advise of possible side effects, I do not expect my practitioner to be able to anticipate all risks or complications.
I understand and acknowledge the Reform's policy to conduct a review (by appointment) every 4 months to re-prescribe medications if necessary.
Infrared Sauna, Ice Bath & Compression Boots
Reform will make every effort to provide a comfortable, clean and safe infrared sauna, ice bath and compression boots environment for you and we ask that you comply with all of our directives and procedures.
You agree to the following on any subsequent usage of our equipment, facilities and services by you in the future at Reform:
You do not have any communicable or infectious disease, illness, open sores or skin disorders.
You are not currently undergoing chemotherapy or radiation treatment.
You are not under the unfluence of drugs or alcohol or non-prescribed medication.
You have received medical clearance which is required by clients who are, but not limited to, pregnant or lactating, have heart disease/cardiovascular conditions, have diabetes, have high/low blood pressure, have implants, have sensitive skin due to poor blood circulation, are haemophiliacs/prone to bleeding, are using medications such as diuretics, barbiturates, beta blockers or anticholinergics.
You are not diabetic with insulin dependency. If you are, you have medical authorisation to part-take in infrared sauna, ice bath and/or compression boots.
Reform DOES NOT RECOMMEND use of the infrared sauna, ice bath or compression boots for the following conditions: Epilepsy, Active Cancer, broken bones or slipped disc, infectious skin lesions / open areas of inflammation, clients with pacemakers or defibrillators, sensitivity to heat or cold, acute pulmonary edema, acute thrombophlebitis, or acute congestive heart failure.
You are at least 18 years of age or have a guardian present to supervise at my appointment.
If pregnant - you have discussed with your health professional and have received medical authorisation to partake in infrared sauna, ice bath and/or compression boots.
You feel well on the day of your appointment and have no incontinence, nausea and in the future, will only partake in infrared sauna, ice bath or compression boots when you feel well.
There is potential for the ground and bathroom floors to be wet and as such slippery due to the ice bath and showers containing water. You waive release and discharge from any and all liability Reform for any slips or falls that may occur on the premises.
You acknowledge that if Reform is required to replace the infrared sauna, ice bath system or compression boots due to a breach in my statements that you will be liable for the full cost of replacement. Examples that would require replacement could be diarrhoea, vomitting, communicable or infectious diseases, uterine membrane rupture, chemotherapy or radiation treatment, water coming into contact with the electrics in the infrared sauna, fake tan staining the timber of the infrared sauna.
An infrared sauna is a dry sauna that uses heat and light to help relax and detoxify the body. Infrared heaters warm the body the same way as natural sunlight. Traditional saunas raise the air temperature to a very high level to warm the body. The infrared sauna, on the other hand, warms the body directly and does not reach temperatures as high as a traditional sauna. Due to environmental conditions outside of our control, we cannot guarantee an exact temperature of the infrared sauna. Temperature of the infrared sauna ranges from 35 - 60 degrees Celsius.
You acknowledge that you will receive the best health outcomes by participating in infrared sauna, ice bath and/or compression boots at least one or two sessions per week.
Packages, Payment and Cancellation Policy
We respect that your time is valuable and we appreciate that you understand ours is too.
All packages purchased are valid for a maximum 6 months from date of purchase. Promotional packages are valid for a maximum of 3 months from date of purchase. Expiration dates and timeframes may be varied from time to time at the discretion of Reform.
We have a 24 hour cancellation policy. Any cancellations that occur more than 24 hours before the scheduled time can be re-booked at no additional cost to the client.
Clients who make a booking and cancel within 24 hours or fail to present for their appointment, may be charged a cancelled booking fee, up to the value of the full consultation fee for that booking.
The cancellation fee is not rebatable through Medicare or any Private Health Funds and Reform do not issue refunds for cancelled appointments.
All clients are liable for any treatment rendered and have been advised of any and all fees applicable to their treatment.
All consultation deposits and fees are payable when making an appointment through a secure online booking system. All costs associated with medications or tests are to be paid when ordered.
In the event a refund is offered we reserve the right to charge an administration fee for any time or cost incurred.
Privacy Policy
Introduction
This section (“Privacy Policy” or “Policy”) provides our policies and procedures for collecting, using and disclosing your information and outlines the security measures we’ve put in place to protect the information that you store using Reform ("Clinical Health Pty Ltd") services, including the services made available through this web site, and any other software or services offered by Reform in connection with such services (the “Services”). By using these Services, you consent to the collection, transfer, processing, storage, disclosure and other uses of your information described in this Privacy Policy.
What information does Reform collect and store?
Personal Information
When expressing an interest in obtaining additional information about the Services or registering to use the Services, Reform requires you to provide your personal contact information, such as your name, company name, address, phone number, and email address (these are referred to below as your “Personal Contact Information”). When purchasing the Services, Reform also requires you to provide financial and billing information, such as billing name and address, credit card number, and the number of employees within the organization that will be using the Services (“Billing Information”).
Data, Diagnostic & Login Information
Using Reform’s Services, you will be able to create, upload, store and share information such as company description, email ID, logo, photos, custom emails, user email IDs, etc. (this is collectively referred to below as “Data”). This information will be stored and maintained on Reform’s web site. If you run into technical errors in the course of using the Services, Reform may request your permission to obtain a crash report along with certain logging information from your system documenting the error (“Diagnostic Information”). Such information may contain information regarding your Operating System version, hardware, browser version (and .NET version information in case of Windows systems), and your email address, if provided. Additionally, certain login information is maintained in a cookie stored locally on your computer (i.e. not on a server) in order to streamline the login process (“Login Information”).
Analytics Information
As you navigate Reform’s website and use our Services, Reform may also collect information through the use of frequently used information-gathering tools, such as cookies and Web beacons (“Website Navigational Information”). Website Navigational Information includes standard information from your web browser (such as browser type and browser language), your Internet Protocol (“IP”) address, and the actions you take on Reform’s website (such as web pages viewed and links clicked). Collectively, this information is referred to as “Analytics Information.”
Third party vendors, including Google, use cookies to serve ads based on a user's prior visits to your website or other websites.
Google's use of advertising cookies enables it and its partners to serve ads to your users based on their visit to your sites and/or other sites on the Internet.
Users may opt out of personalized advertising by visiting Ads Settings or by visiting www.aboutads.info.
Geo-Location Information
Reform does not collect any information regarding your real-time geo-location while using the Services; however, it may do so at some point in the future. We will request your permission before collecting such information.
What does Reform do with the information it collects?
Reform uses the information it collects in the following ways:
Personal Contact Information – We use this information primarily to administer our Services to you and provide you with updates and product announcements. Per the Privacy and Conditions, we may use some of your information for marketing purposes, as explained below.
Billing Information – Reform does not store any Billing Information on its servers. Instead, we third party payment providers, to store and process all payment related transactions.
Data, Diagnostic Information and Login Information – We use this information solely for the purpose of administering and improving our Services to you.
Analytics Information – Reform may use your Analytics Information in conjunction with an analytics service such as Google Analytics to monitor and analyze use of the Services, for the Services’ technical administration, to increase the Services’ functionality and user-friendliness, and to verify users have the authorization required for the Services to process their requests.
Sharing & Disclosure of Private Information
Third Party Applications and Your Use
Reform provides users with the ability to link to their Data on third party sites such as Facebook, Twitter and LinkedIn. Such linking is at the complete discretion of users. Because of this, Reform cannot be held responsible or liable for the linking of user’s Data to such third party sites, nor for how these third party sites use such links.
Marketing and Publicity
Per our Privacy and Conditions, you agree to permit Reform to identify you as a customer and to use your name and/or logo in Reform’s website and marketing materials.
Sale of Personal Information
Reform does not sell, rent, or trade your private information to any third parties in any way.
Service Providers and Business Partners
Reform may use certain trusted third party companies and individuals to help us provide, analyse, and improve the Services (including, but not limited to, data storage, maintenance services, database management, web analytics, payment processing, and improvement of the Services’ features). These third parties may have access to your information strictly for the purposes of performing these tasks on our behalf and under obligations similar to those in this Privacy Policy.
Non-Private or Non-Personal Information
We may disclose your non-private, aggregated, or otherwise non-personal information, such as usage statistics of our Services, in our discretion.
Our Use And Disclosure of Information
Reform will not disclose any personally identifiable information about any individual except as set forth in this Privacy Policy. This applies to information about our customers and information our customers provide to us about their customers. We are not limited in any way in our use of non-personal information that does not permit direct association with any specific individual or non-identifiable aggregate information about our users (such as the number of customers who use our services, the geographic distribution of our users, the amount of information located and/or removed, etc.).
Internal Uses of Your Personally Identifiable Information
We collect, store and process your personally identifiable information on servers located in the United States. Due to the unpredictable nature of Internet routing, your information may pass through other countries while in transit to our servers. We use the information we collect about you in order to:
- Develop and deliver our services
- Process your transactions
- Provide customer service and manage your account
- Improve our products, services and marketing.
We provide access to personally identifiable information about our users only to those who require it for the above purposes.
Reform will not sell or rent any of your personally identifiable information to third parties. Reform will not share any of your personally identifiable information with third parties except in the limited circumstances described below.
We share information with service providers under contract who help with our business operations such as payment and order processing, fraud investigation, bill collection, and information management and analytics. If content generation is included in your services, we may share information with service providers under contract to create, edit and/or publish such content. These third parties are obligated to protect your information and are contractually prohibited from using your personally identifiable information for any other purpose. They are never permitted to share your information with any third parties. They are authorized to use your personal information only as necessary to provide these services to Reform.
We disclose information that we, in good faith, believe is appropriate to cooperate in investigations of fraud or other illegal activity, to conduct investigations of violations of our Privacy of Use and/or to protect our right, protect your safety and the safety of others. For example, this means that if we conduct a fraud investigation and conclude that one side has engaged in deceptive practices, we reserve the right to provide that person or entity’s contact information (but not bank account or credit card information) to victims who request it.
We disclose information in response to a subpoena, warrant, court order, levy, attachment, order of a court-appointed receiver or other comparable legal process, including subpoenas from private parties in a civil action. If the subpoena seeks information about an identified subscriber or limited group of subscribers, we will make reasonable business efforts to contact the subscriber(s) before providing information to the party that requests it. We cannot guarantee that we will be able to do so in all cases, whether due to a time limit, court order, inability to effectively contact a subscriber, or other circumstances.
When a user signs up for a co-branded version of our service through links to reformclinics.com.au from our co-branded partner’s website, Reform will share with the co-branded partner that user’s name, e-mail address and physical address in order to provide enhanced integration between Reform’s services and the services of our co-branded partner. If you do not want your information shared with Reform’s co-branded partner, sign up for Reform directly through reformclinics.com.au and other sub-domains and not through a link from our partner’s website.
We disclose information to your agent or legal representative (such as the holder of a power of attorney that you grant, or a guardian appointed for you).
We share information with companies that provide public relations and marketing services for us. Such information will only be shared by us to customize, measure and improve our products, services and advertising. It will not be shared with third parties for their marketing purposes. These third parties are contractually obligated to protect your information and are prohibited from using your personally identifiable information for any other purpose.
As with any other business, it is possible that in the future, Reform could merge with or be acquired by another company. If such an acquisition occurs, the successor company would have access to the information maintained by Reform, including customer account information, but would continue to be bound by this Privacy Policy until it is amended.
We share your information with our parent, subsidiaries and joint ventures to help coordinate the services we provide to you, enforce our terms and conditions, and promote trust and safety.
The implementation of our Services, by its very nature, may require using your personally identifiable information to locate other information about you. Such use may include, but not be limited to, using your information to search the publicly accessible Internet sites as well as searching private information databases and sites.
The implementation of our Services, by its very nature, may require revealing your personally identifiable information in order to effect removal of Internet content about you. For example, we may have to disclose your name to a website in order to notify them to remove Internet content about you. This occurs with your express permission for a specific, given purpose.
Internal Uses of Your Personally Identifiable Information
We will retain your information for as long as your account is active or as needed to provide you the Services. If you wish to cancel your account or request that we no longer use your information to provide you the Services, you may delete your account (this will be done by Reform’s customer care team). If you delete your account, your Data will no longer be stored in our servers. While we try to delete your Data from our servers as quickly as possible, please be aware that there may be a delay from the time you delete your account to the time that your Data is removed, and that some of your Data may continue to exist for a period in backup copies.
Your data in the case of an event
In the event that Reform goes through a business transition, such as a merger, an acquisition by another company, or a sale of a portion of its assets, users' personally identifiable information will, in most instances, be part of the assets transferred. Users will be notified via prominent notice on the site for 30 days after a change of ownership or control of their personally identifiable information. If, as a result of the business transition, a user’s personally identifiable information will be used in a manner different from that stated at the time of collection, users will be given a choice consistent with our notification of changes section.
Internal Uses of Your Personally Identifiable Information
If you reside in the European Union (“EU”), United Kingdom, Lichtenstein, Norway, Iceland or Switzerland, you may have additional rights with respect to your personally identifiable information (otherwise known as Personal Data). These rights may include rights under the EU’s General Data Protection Regulation (“GDPR”), if you are a resident of the EU, United Kingdom, Lichtenstein, Norway or Iceland. “Personal Data” is any data that relates to an identified or identifiable natural person. Examples of Personal Data include identifiers such as name, location data, and unique online identifiers.
In addition to the principles, practices and policies set forth above in this Privacy Policy, Reform has adopted the following principles to govern its collection and processing of Personal Data:
Personal Data shall be processed lawfully, fairly, and in a transparent manner.
The Personal Data collected will only be those specifically required to fulfil Reform’s obligations to deliver the Reform service.
Personal Data shall only be retained for as long as it is required to fulfil contractual requirements.
Personal Data shall be adequate, relevant, and limited to what is necessary in relation to the purposes for which they are collected and/or processed. Personal Data shall be accurate and, where necessary, kept up to date.
The data subject has the right to request from Reform access to and rectification or erasure of their Personal Data, to object to or request restriction of processing concerning the data, or to the right to data portability. In each case such a request must be put in writing to Reform.
Community
Our Services may include publicly accessible community services such as blogs, forums, and wikis. Please be aware that any information you provide in these areas may be read, collected, and used by others who access them. Your posts on these communities may remain even after you cancel your account.
